    Gov. Jerry Brown has thrown his support behind expanding Medi-Cal, the health insurance program for impoverished Californians, to the full extent authorized by the 2010 federal healthcare reform law. It was the right choice, and Brown deserves credit for recognizing that the benefits to public health and the economy outweigh the potential costs. But his <a href="http://lat.ms/13ENuYr">budget proposal</a> left state lawmakers to decide whether to keep responsibility for the expanded program in Sacramento or hand it off to the counties. And while it's worth reevaluating how to pay for the medically indigent, it would be a disaster to transfer so much of Medi-Cal's duties to ill-prepared local authorities.

    For all of our sophisticated medical care, Americans can expect shorter lives and more health troubles than the people of other well-off nations, according to a new report. And that's not just true of infants and poor people, the groups usually pinpointed as particularly vulnerable to health issues; it is also the case for the affluent, teenagers and middle-aged people. Some of this can be traced to a lack of preventive and primary care, some to car accidents and violence, some to obesity and poor health habits. The United States clearly cannot rest on its past laurels; nor can it expect medical laboratories and research hospitals, for all the lifesaving work they do, to solve the problem.

    Although Republicans are eager to repeal the entire 2010 healthcare reform law, they started the new session of Congress last week by taking aim at one provision in particular: the Independent Payment Advisory Board, a yet-to-be-named group of 15 presidential appointees from various healthcare disciplines that could play a key role in limiting the growth of Medicare spending. Critics argue that it's a bad idea and even un-American to put so much power in the hands of unelected bureaucrats. But with lawmakers seemingly unable to resist the pressure from the healthcare industry to spend freely on Medicare, enlisting the help of independent experts may be the only way to hold down costs.

  19. Medicare: Is 67 the new 65?

    Looking for ways to slow the growth of entitlement programs, budget negotiators in Washington are considering making seniors wait two years longer to qualify for Medicare &mdash; from age 65 to 67. Many Republicans have endorsed the idea, noting that Medicare beneficiaries now live far longer on average than they did when Congress created the program in 1965. The problem with the proposal is that it wouldn't save the federal government much money overall, even though it might cut Medicare's costs. Worse, it would probably cause total spending on healthcare to go up faster, which is the opposite of what Washington should be trying to achieve.
