  11. Director of Lebanon VA Hospital subject of investigation

A report from the Department of Veterans Affairs says Robert Callahan, Jr., the director of the VA Hospital in Lebanon, failed to immediately report an investigation into the possible criminal activity of a VA Hospital nurse. That nurse was arrested for diverting drugs, oxycodone and morphine, intended for a patient, by VA Police last June. A coworker noticed the nurse was ordering the 'as-needed' medications for a particular patient that didn't require them when other nurses were on duty. Callahan did request a VA Police investigation that resulted in the nurse's arrest 8 days later. However, guidelines required him to immediately report possible felony matters to the Office of the Inspector General.
