Richard D. Koshak March 10, 1943 - March 26, 2014

Richard D Koshak, 71, of Burbank California passed away at his home. He is survived by his sister Cathy, children Richard, Jill, and James, as well as seven grandchildren. Memorial services will be held Saturday April 5, 2014 2:30 pm at the Old North Church in Forest Lawn Hollywood Hills. Memorial contributions can be sent on his behalf to St. Jude.
