Zuma satellite plunged after SpaceX launch because of Northrop Grumman errors, report says
The loss of the secretive Zuma satellite — which was launched by SpaceX in January — was reportedly due to a problem with a part modified by Northrop Grumman Corp. that attaches the payload to the rocket. (April 9th, 2018)
