Alleged Health Scam Halted: The state Insurance Department said it shut down an allegedly fraudulent health insurer that operated out of an Ontario post office box and was collecting premiums of more than $200,000 a month for worthless medical coverage. The company, United Healthcare Benefits Trust, and its founder, Tony Ullah, 46, of Upland, originally were ordered to halt operations in March, 1990. However, claiming to be exempt from state jurisdiction, the firm obtained an injunction blocking the order. A federal judge last week dissolved the injunction, clearing the way for the shutdown. The Insurance Department said United Healthcare had about 1,500 customers, including small businesses and individuals.
