The National Transportation Safety Board issued urgent nationwide safety recommendations about the train control system involved in the Metro crash that killed nine people and injured 80 in June.

Investigators said that errant signals from equipment in a train control room gave a false signal to the automated crash-avoidance system, erroneously telling it the track was clear when it was occupied by an idling train.

Transit agencies have been told to examine their train control systems, in cooperation with the manufacturer, to guard against further malfunctions.
