The redesign of the space shuttle is not complete and, despite the resumption of shuttle flights, NASA is in danger of forgetting “lessons learned” as a result of the 1986 Challenger explosion, a National Research Council panel warned. The independent scientific panel was formed at the behest of the White House-appointed Rogers Commission to monitor the space agency’s compliance with its recommendations. The warnings came in its final report, which detailed nine problem areas, only one of which the National Aeronautics and Space Administration is committed to solving.
