As proof of a child’s birth date, parents must provide a certified copy of the child’s birth record, a local registrar’s statement, a county record certifying the date of birth, a baptism certificate duly attested, a passport, an immigration visa or a notarized affidavit from the minor’s parent, guardian or custodian when no other document is obtainable.

State law also requires that first-time students be immunized for polio, red measles, diphtheria, tetanus, whooping cough, German measles and mumps. Parents must present the school with written documentation of the immunizations, signed by the medical provider.

School immunizations are available from private doctors, at regional clinics of the Orange County Health Care Agency or by appointment from the district’s Assessment and Registration Center at Cook Elementary School, 9802 Woodbury Road, Garden Grove.

The immunization clinic at the registration center is open to district students only. Free immunizations will be administered between 8 a.m. and 2 p.m. weekdays from Monday through Sept. 9, excluding Labor Day. The clinic is closed for lunch between noon and 12:30 p.m. Call (714) 663-6207 for an appointment.

Additional information is available by calling the district’s public information office at (714) 663-6503.

The Garden Grove Unified School District serves most of Garden Grove and portions of Santa Ana, Anaheim, Westminster, Cypress, Stanton and Fountain Valley.
