* You can request a disclosure of your medical history by calling the Medical Information Bureau--which is like a credit reporting agency--if one has been compiled on you. If you spot errors in your history, you can seek corrections. Call the Medical Information Bureau, (617) 426-3660.

* If you are in an HMO, file complaints with the Department of Corporations health-care service plan division, (916) 324-9020.

* For all other health insurance complaints, file a complaint with the state Department of Insurance Consumer Services Division, 300 S. Spring St., Los Angeles, Calif. 90013. Or call (800) 927-HELP.
