The death toll rose to at least 128 in the chain-reaction crash of three passenger trains in Pakistan, authorities said.

The country’s worst rail disaster in more than a decade also injured hundreds at a remote station near the town of Ghotki.

Abdul Wahab Awan, general manager of Pakistan Railways, blamed the driver of the Karachi Express for misreading a signal and rear-ending the Quetta Express. Derailed cars were then hit by the Tezgam Express.
