IMMUNIZATION REQUIREMENTS

In order to enter or transfer into child-care centers, public and private elementary and secondary schools, children under the age of 18 years must meet the following California immunization requirements: INSTITUTION: Child care center, day nursery, nursery school development center AGE: 2-3 months VACCINE: Polio. TOTAL DOSES: 1 dose VACCINE: DTP. TOTAL DOSES: 1 dose INSTITUTION: Child care center, day nursery, nursery school development center AGE: 4-5 months VACCINE: Polio (OPV). TOTAL DOSES: 2 doses VACCINE: DTP, or combination of DTP and diphtheria-tetanus toxoids. TOTAL DOSES: 2 doses INSTITUTION: Child care center, day nursery, nursery school development center AGE: 6-14 months VACCINE: Polio (OPV). VACCINE: 2 doses* VACCINE: DTP, or combination of DTP and diphtheria-tetanus toxoids. TOTAL DOSES: 3 doses INSTITUTION: Child care center, day nursery, nursery school development center AGE: 15-17 months VACCINE: Polio (OPV). VACCINE: 2 doses* VACCINE: DTP, or combination of DTP and diphtheria-tetanus toxoids. TOTAL DOSES: 3 doses INSTITUTION: Child care center, day nursery, nursery school development center AGE: 18 months-4 years VACCINE: Polio (OPV). TOTAL DOSES: 3 doses* VACCINE: DTP, or combination of DTP and diphtheria-tetanus toxoids. TOTAL DOSES: 4 doses VACCINE: Measles, rubella and mumps. TOTAL DOSES: 1 dose of each, separately or combined, on or after the first birthday (preferably at 15 months) INSTITUTION: Child care center, day nursery, nursery school development center AGE: 18 months VACCINE: Haemophilus influenzae type B (Hib) vaccine TOTAL DOSES: Not legally required by the state but strongly recommended; parents should consult their physician or local health department INSTITUTION: Elementary school at kindergarten level and above AGE: 4-5 years VACCINE: Polio (OPV). TOTAL DOSES: At least 3 doses** VACCINE: DTP, or combination of DTP and diphtheria-tetanus toxoids, Measles, rubella and mumps. TOTAL DOSES: At least 4 doses**, 1 dose of each, separately or combined INSTITUTION: Elementary school, secondary school AGE: 7-17 years VACCINE: Polio (OPV). TOTAL DOSES: At least 3 doses** VACCINE: diphtheria and tetanus toxoids given as DTP, DT or Td (pertussis not required). TOTAL DOSES: At least 3 doses** VACCINE: Measles, rubella (mumps not required). TOTAL DOSES: one dose of each, separately or combined, on or after the first birthday (preferably at 15 months INSTITUTION: Elementary school, secondary school AGE: 18 years and older VACCINE: None NOTE: (OPV) is oral polio vaccine. No vaccinations are required for children younger than two months. For additional information, contact local health department.

* If inactivated polio vaccine or a combination of IPV and OPV is used, one more dose is required. At the elementary and secondary school levels, one more dose is required if the fourth or last dose was given before the second birthday.

** One additional dose if the last dose was given before second birthday.
