A breakdown in air traffic control procedures occurred during an evening rush of planes out of Chicago two weeks ago in which a supervisor’s concerns about overcrowded airspace went unheeded and controllers mixed up two jetliners, officials disclosed. Confusion over radar identity tags given United Airlines and American Airlines jets caused the American pilot to turn and head toward an incoming American Boeing 727 before veering off course, investigators at the National Transportation Safety Board said.
