Pilot error caused the crash of a minesweeping helicopter during a training exercise in dense fog off the Golden Gate that killed all eight crewmen last July, the Navy reported. Investigators concluded that the tail rotor of the craft struck the water when the pilot, Marine Capt. Douglas Durre, hovered too close to the surface. There was evidence of “overaggressiveness on the part of Capt. Durre . . . (a trait that) may have led him to continue the mission in adverse weather conditions,” the Navy said.
