Two Northern California nursing homes have been cited and fined for the deaths of patients.

The state Department of Health Services issued an “AA” citation, its most severe, and a $60,000 fine to the Evergreen Health and Rehabilitation Center of Petaluma. The citation is linked to an incident two years ago in which a 76-year-old patient caught fire while smoking in her wheelchair. She died a day later.

Evergreen administrator Vince Hambright said the citation was being appealed.

The Pleasant Care Convalescent facility in Ukiah also received an “AA” citation and $60,000 fine in the death of a 54-year-old male patient.
