The Navy has opened an investigation into a weekend accident in which two men died in the crash of a trainer plane during what may have been an unauthorized flight, Pentagon sources said in Washington. The investigation was opened after the crash of a Navy T-34B Mentor shortly after takeoff from the airport at Empire, Nev., about 80 miles northeast of Reno. The plane, a small single-propeller aircraft, was piloted by Lt. Anthony G. Gragg, 28, of San Leandro, Calif. Also killed in the crash was William F. Cornwell, 39, a civilian Navy employee at the Naval Air Rework Facility in Alameda, Calif.
