UC Irvine Medical Center: An article in Friday’s LATExtra section about state inspectors forcing UC Irvine Medical Center to fix “medication management” problems said that once investigators issue a deficiency report, hospital officials have up to a month to respond with a plan of correction. Hospital officials generally have 10 days to respond.

Southwest Chamber Music: The caption on one of the photos with an article in Friday’s Calendar about Pasadena-based Southwest Chamber Music visiting Vietnam referred to the opening concert taking place in Ho Chi Minh City. The opening concert in the Ascending Dragon Music Festival was in Hanoi.

Lehman report: An article in the Friday Business section about a court-appointed examiner’s report on the 2008 collapse of investment bank Lehman Bros. misspelled the last name of the examiner, Anton Valukas, as Volukas.
