Chevron refinery fire caused by pipe

A January fire at the Chevron refinery in Richmond, Calif., was sparked by a corroded pipe that should have been detached 20 years ago, investigators said.
The failed pipe leaked during a Jan. 15 maintenance operation and caused the fire, prompting thousands of people to take shelter and releasing more than 400 pounds of toxic gas.
The San Ramon, Calif.-based company said it had replaced all faulty equipment discovered during the investigation.
