Errors in judgment by a San Diego-based squadron commander and flight crew caused a jet to plunge off the flight deck of the Constellation last year, according to a Navy report obtained by the San Diego Union-Tribune.

The two pilots aboard the S-3B Viking safely ejected during the April 2003 mishap, but the $27-million aircraft sank in the Persian Gulf.

An investigation found Cmdr. Sean Kelly, the squadron’s commanding officer, failed to recommend that the aircraft, which had developed hydraulic problems, be towed after landing.
